How is a BOV different from an appraisal?

A BOV is a broker's written opinion of market value based on comparable sales and leases. An appraisal is a formal valuation by a licensed appraiser, usually required by a lender, and costs money. A BOV is not an appraisal.

Will my information stay private?

Can I keep operating in the building after selling?

Often, yes. A sale leaseback lets you sell the property and stay on as a tenant. Mention it in the form if that interests you.
